Professional Liability Insurance protects you against claims arising from real or alleged errors or omissions, including negligence, in the course of your professional duties. You pay only for the principals, employees or additional insureds, there is no extra premium for the coverage of your business. Additionally, there is no premium charge for coverage provided for opticians, technicians and assistants.
Select the coverage limits that suits your needs, up to $2,000,000 per incident, $4,000,000 annual aggregate. High Deductible Health Plans and Health Savings Accounts
Open a Health Savings Account if you already have a qualified high deductible health plan. Sign up online with HSABank.com In order to open a Health Savings Account, you must satisfy these four eligibility guidelines: 1) you must be enrolled in a qualified high deductible health plan (not just a health plan with a high deductible), 2) you may not be covered under any health plan that is not a high deductible health plan, 3) not be enrolled in Medicare, and 4) not be claimed as a dependent on another person’s tax return. Significant premium savings are available through a number of qualifying high deductible health plans (HDHP) on an individual or group basis. Blue Cross Programs
For Individuals and Small Groups (2 50) You have a choice of health care programs for your family and your employees. Programs include: PPO, HMO and HSA. HMO Plans: • Low out-of-pocket costs and unlimited lifetime benefits for covered services. • Minimal co-pays for office visits and self-referral for OB/GYN (women's health specialists). • Specialty services include allergy, dermatology and ear, nose and throat. PPO Plans: • Direct access to the doctors, hospitals and specialists of your choice. • Immediate (deductible-free) benefits on some plans for office visits and generic drugs. • Low negotiated provider fees that substantially reduce your out-of-pocket costs. |

This program lets you know exactly how much you're covered for in advance. A table of allowances will detail how much the program covers for each dental service. You are responsible for the difference between the allowed dollar amount and the dentist's filed fee, when using a Delta dentist (93% of California's dentists are Delta dentists.) If you are not using a Delta dentist, you are responsible for the portion of the fee not covered by the allowance, up to the billed amount.

| You may apply for up to $500,000 of benefits at competitive group premiums. Your spouse and employees are eligible for up to $500,000. Plan features include coverage for children. | This plan offers permanent insurance (up to $1,000,000) that is guaranteed renewable to age 95 and an optional cash accumulation account that earns tax-deferred interest at competitive rates (minimum guarantee: 4%). Long Term Disability coverage helps protect your income in the event you become totally disabled.

The plans include Waiver of Premium and a choice of waiting periods (90 or 180 days). Additional benefits include Rehabilitation Benefit and Survivor Benefit. |

Pays the normal and fixed expenses of running your office while you are disabled so you don't have to use your personal disability income.
